Bankruptcy Forms Revised

Effective December 1, 2015, most Official Bankruptcy Forms were revised, reformatted, and renumbered. 

For creditors asserting claims, the Proof of Claim form, formerly Official Form B10, is now Official Form B410. The Mortgage Proof of Claim Attachment for residential mortgage holders is Official Form 410A, and the supplements for residential mortgage holders, Notice of Mortgage Payment Change and Notice of Postpetition Mortgage Fees, Expenses, and Charges, are now known as Official Forms B410S1 and B410S2, respectively. Reaffirmation agreement forms have changed as well.
